Code of Primary Health Networking - 
Statement of Purpose for Active Members

     Membership of Vietnamese Primary Health Care Network (VPHCN) is open to any Vietnamese General Practitioner in Fairfield Local Government Area (FLGA) interested in primary health care for the Vietnamese community in the local area.

Membership of VPHCN is free. As a member of VPHCN you will:

• Receive support for timely referral to appropriate services for clients of GPs within the Primary Health Care Network

• Facilitate use of case plan, case conferencing and general practice

• Participate in all of the Vietnamese Primary Health Care activities at NO cost

• Receive free information about education and training events

• Receive the bi-monthly newsletter and access to the VPHCN’s website for more information on services and programs available.

For the benefit of the Vietnamese community in FLGA and as a member of VPHCN you are encouraged to:

• Become actively involved in the Vietnamese Primary Health Care Network by referring your clients to Primary Health Care / Social Network services

• Give feedback on services provided to the VPHCN Project officer

• Attend Health Forums to discuss any issues related to the health of the Vietnamese Community in the Fairfield LGA.
