Primary Health Care Network
Evaluation, July 2004 The
Vietnamese Primary Health Care Network Project started in April 2003. The
Network is designed to make it easier for GPs to refer to Fairfield Health
Service and other health services in Fairfield. You have involved with the
Network in April 2003. Funding
for the network was due to end in September 2004. However the Fairfield GP Division is keen to keep it going
and will continue the funding. We
are currently evaluating the Network so far and would welcome your
feedback about your experience of the Network, and any suggestions you may
have for improving it in the future. It will take about 10 minutes of your
time to fill in this questionnaire. This
evaluation will form part of an independent evaluation conducted by the
Department of Health of the Fairfield Network and 5 other primary health
care networks. You are welcome to make any additional comments to the
independent evaluator, Dr Ida Vincent. Dr Vincent can be contacted on 4861
2555. Any
comments you make will be treated in confidence. 1.
